Note: Weight Room & Gymnasium Rules:

By completing the information below and signing the reverse side of this form, the participant and/or guardian agree to the following rules and understand the failure to obey these rules or display conduct considered detrimental to the overall enjoyment of the facility for others will result in dismissal from the facility and possible permanent expulsion:

1. MEMBERSHIP CARDS MUST BE PRESENTED EVERY TIME YOU WISH TO ENTER THE CB HUSS RECREATION CENTER FACILITY.
2. No on under the age of 9 years is allowed in the CB Huss Recreation Center unsupervised.
3. No Profanity
4. No smoking, smokeless tobacco, e-cigarettes or vapes are allowed on the property.
5. No drugs allowed.
6. Registered sex offenders are not allowed to be on the property.
7. Any equipment checked out for the gymnasium or the game room must be returned immediately after use.
8. No sitting on tables in the game rooms or gymnasium.
9. All trash must be placed in trash cans.
10. No food or drink other than bottled water is allowed in the gymnasium.
11. No music allowed in the gymnasium.
12. Weight room and cardio room equipment must be wiped off after every use.
13. Weights must be re-racked in the weight room.
14. No excessively loud or inappropriate music is allowed in the weight room.
15. Cramerton Parks and Recreation programming takes precedence over all open play.
16. All users must follow and obey the requests of the Recreation Center Supervisor or any other Parks and Recreation Staff at all times.
